The Applicant's signature attests to the financial responsibility and willingness to pay our invoices in accordance with our credit terms as stated on the invoice. Terms are NET 15 days. Finance charges will incur on past due balances. Applicant agrees to pay all of the seller's cost of collection, reasonable attorney's fees and court costs, as fixed by the court, in the event of any legal action is required to collect any indebtedness owed by the Applicant to Independent Equipment Repair (IER) LLC.

By witness of my signature I authorize Independent Equipment Repair (IER) LLC and it's representatives to conduct inquiry into the references listed or attached, and furthermore authorize these listed references to release information requested to Independent Equipment Repair (IER) LLC for the purpose of establishing credit. A 4% convenience fee will be added to any invoice for payment made by credit card.
